Les cancers digestifs sont l'une des principales causes de d\u00e9c\u00e8s li\u00e9s au cancer dans le monde, et partagent un pronostic sombre aux stades avanc\u00e9s, avec une survie globale m\u00e9diane allant de 10 \u00e0 30 mois. Les th\u00e9rapies cibl\u00e9es et les immunoth\u00e9rapies, les deux avanc\u00e9es majeures r\u00e9centes dans le traitement du cancer, n'ont conduit qu'\u00e0 des am\u00e9liorations mineures dans l'arsenal th\u00e9rapeutique des cancers digestifs avanc\u00e9s. Par cons\u00e9quent, il est urgent de d\u00e9velopper des approches alternatives de m\u00e9decine personnalis\u00e9e pour ces patients.<\/span><\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

Les organo\u00efdes sont des structures tridimensionnelles multicellulaires d\u00e9velopp\u00e9es in vitro, qui conservent les caract\u00e9ristiques g\u00e9notypiques et ph\u00e9notypiques de leur tumeur d'origine. Cela sugg\u00e8re que les organo\u00efdes d\u00e9riv\u00e9s de tumeurs des patients (AOP) pourraient pr\u00e9dire la r\u00e9ponse au traitement des cancers digestifs avanc\u00e9s et \u00e9clairer la d\u00e9cision clinique sur la meilleure option th\u00e9rapeutique pour chaque patient.<\/span><\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

Les organo\u00efdes, avatars ex vivo d'organes normaux et pathologiques, ont fait l'objet d'un immense int\u00e9r\u00eat dans la recherche fondamentale et translationnelle. Ils pourraient aussi \u00e9ventuellement \u00eatre utilis\u00e9s en clinique dans un but de m\u00e9decine personnalis\u00e9e. Nous proposons un essai clinique multicentrique innovant \u00e9valuant la faisabilit\u00e9 et l'efficacit\u00e9 des organo\u00efdes utilis\u00e9s dans le traitement personnalis\u00e9 des patients atteints de cancers digestifs r\u00e9fractaires.<\/p>

ORGANOTREAT-01 testera l'hypoth\u00e8se que les AOPs peuvent permettre de g\u00e9n\u00e9rer des chimiogrammes personnalis\u00e9s et d'identifier des options th\u00e9rapeutiques inattendues pour les patients atteints de cancers digestifs r\u00e9fractaires. ORGANOTREAT-01 est une \u00e9tude prospective, multicentrique, de phase I \/ II qui commencera \u00e0 inclure des patients en 2021. Les AOPs sont g\u00e9n\u00e9r\u00e9s \u00e0 partir de biopsies ou de chirurgies tumorales et sont trait\u00e9s avec diff\u00e9rents m\u00e9dicaments, seuls ou en combinaison, pour \u00e9tablir le chimiogramme de chaque patient .<\/span><\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t

Our primary objective is to evaluate the feasibility of PDO-based precision medicine. Patient will be biopsied at Gustave Roussy during their last line of treatment. The biopsy specimen will allow the production of organoids that will be challenged with 26 therapeutic compounds and the personalized chemogram report will be released to the board of experts.<\/span><\/p><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t\t
